Members Application Process
Approximate length of process:
60-90 days to join the Lakeshore Health Network

Required Fees:
$100 LHN/MAPA Membership
$150 MHP Medical Staff Dues
$75 Allied Health Application Fee
- Physicians must have admitting privileges at Mercy General Health Partner’s hospital to apply for membership into Lakeshore Health Network.
- MHP Medical Staff Department will provider a pre-application to be filled out and is reviews by the Vice President of Medical Affairs. Upon approval of the pre- application the Initial application will be sent to the provider.
- Once the initial application is returned to the MHP Medical Staff office the department then will do primary source verification. The application is then presented to the Credential Committee for review and recommendation. That same day Lakeshore Health Networks Medical Management Committee will review the application.
- Following the Board approval, you will receive a call from Westshore Health Networks Provider Relations Department to sign a participation agreement. This agreement must be signed and returned to Westshore Health Network.
- Once the agreement has been returned Lakeshore Health Network credentialing department with expedite the credentialing with the plans
- You will receive notification when you are active with the plan.
